Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

By law, we must take steps to keep all of your personal and health information private. Recent changes in the law require that we now tell patients when we keep their records private, and when we might give information to other parties, such as doctors, insurance companies, Medicaid, or Medicare. We have not changed the way we handle your health information. The new law simply requires that we tell you our policy.

When you choose a pharmacy, any pharmacy, that pharmacy must obtain from you basic information such as name, address, phone number, date of birth, drug allergies and insurance information. Just like any other pharmacy, we also keep all of your prescription records on file in our computers. We secure our computer systems and require a password for their use. We keep our pharmacy locked at all times. Printed or written records are stored so others cannot have access to them.

Section One - Ways we might disclose your health information

Treatment - Like any pharmacy, we will talk with your doctors, nurses or other health care team members to get prescriptions over the phone, by fax or in writing.

Payment - Like any pharmacy, we must provide information about your prescriptions to Medicaid, Medicare or your insurance company for them to pay us.

Operations - Like any pharmacy, we are required to print reports to keep on file. State and federal laws tell us we must do this.

As required by law or law enforcement officials - Like any pharmacy, we might have to provide information to law enforcement officials. This could happen, but rarely ever is required of pharmacies.

It might be necessary for us to talk with your family members - You might ask a family member to order prescriptions or talk to us about payment. We might call your home and talk with a parent or spouse. If you do not want us to do this, please tell us in writing. We will always exercise caution when talking with any family member other than you.

Section Two - Ways we will NOT use your health information

We will not provide your confidential information to sales people, marketing companies or any similar parties unless you ask us to, in writing. Our staff understands that your health information should be kept private.

Section Three - Your health information rights

You can ask for restrictions in the way your health information is used. While pharmacies are not required to agree, we will make every reasonable effort to accommodate your wishes.

You have the right to ask for a copy of your prescription records. You also have the right to ask us to correct any errors in your records. You have the right to ask for a list of those to whom we have provided information about you, other than those parties we described in Section One.

Section Four - Changes to our policy

Health care providers have the right to change their Privacy Policy. If we change our policy, we will provide our patients with a printed copy. If the law changes and we are required to tell you, we will also do that.

If you have a complaint, we will work hard to address your concerns. Please contact us immediately and inform us of the issue. Our goal is to resolve your complaint to your satisfaction.

The federal government has an agency that handles unresolved complaints. They ask that you work with your doctor, pharmacy, hospital or other health care provider first. After that, you can contact them at:
